The first home visit connects discharge advice to the person’s real environment. The physiotherapist can look at how the person gets out of bed, reaches the bathroom, stands from their main chair and moves between the rooms they need during the day.

“Getting stronger” becomes easier to follow when it is tied to a purpose. That might mean making breakfast with less help, reaching a parked car, managing the entrance or having enough energy left for personal care. The plan should respect medical precautions and the pace of recovery rather than impose a fixed timetable.

How do I arrange physiotherapy after hospital discharge?

How soon after discharge can I book?

You can enquire before or after discharge. The safe start date depends on medical instructions, current symptoms, the home address and clinician availability.

Do I need the hospital exercise sheet?
