How this service can help
A useful plan starts with the older person’s preferences and current routine. Family observations and staff handover can add context, but they do not replace the person’s voice or the need for consent and respectful communication.
The physiotherapist may be asked about mobility after a fall, changes in walking, strength, pain, equipment or maintaining participation in daily life. In a residential setting, the visit also needs to fit facility access, infection-control, documentation and communication processes.
Movement and participation
Choose goals connected to daily activities that matter to the resident rather than a generic exercise list.
Falls and equipment
Assess relevant movement, walking-aid use and environmental factors, and identify when another discipline should be involved.
After a health change
Review function after hospital, illness or a fall within the person’s medical advice and care plan.
Clear reporting
Agree who needs updates, what the funding body requires and how information can be shared lawfully.

Common questions
Can you visit a residential aged care home?
A visit may be possible with resident consent, facility approval, a suitable clinical request and route availability. Contact us with the facility and key contact.
Can a care manager refer?
Yes. Use the referral form and include goals, relevant history, consent, funding approval, reporting requirements and the best contact for scheduling.
Will family receive an update?
With the older adult’s consent or other lawful authority, we can agree on who receives booking and clinical information.
Can you provide ongoing maintenance visits?
Frequency is based on assessed needs, goals, response, funding and whether physiotherapy remains the appropriate service. It is reviewed rather than guaranteed indefinitely.
What aged care rules apply?
Funding approvals, service agreements and provider responsibilities vary. The Aged Care Quality and Safety Commission explains the strengthened Quality Standards and Statement of Rights.
